The Media Type 0 and 1 = unknown have me a bit puzzled (maybe it’s not an mp4 file - just something else renamed?) This is not exactly a “here’s the solution” post, but you may get some ideas for things to try from what I’m writing below.
Usually I try to play mp4 files with QuickTime Player (the mp4 container was derived from the mov container, so it makes sense QuickTime - the official (though not necessarily the best) player for mov should be able to play mp4 too.
A lot of people don’t like QuickTime because the player installs a lot of junk (mostly shortcut icons, associates file types without your consent, and has the stupid content guide by default). All this can be remedied by tweaking the settings, but most people can’t be bothered. That’s why they turn to QuickTime Alternative. However my experience with QTA has been mixed. It is without a doubt better at rendering, however it doesn’t play all files correctly - the most notable exception is many mp4 files (Media Player Classic).
I’ve noticed recently a subset of mp4 files don’t play on QuickTime Player either. Those will play fine with VLC Media Player though. I’m not sure why, however I suspect they may be encoded with non-standard tools, or be non MPEG-compliant.
